Live Oak Health Center is under the supervision of a licensed Nursing Facility Administrator, a Medical Director, and Professional Nurses. The day-to-day operation and supervision of nursing care is managed by the Director of Nursing and the Assistant Director of Nursing. These qualified professionals each contribute to insure the best quality of life for our residents.

  • The Health Care Team makes a pre-admission need assessment.
  • Our round-the-clock coverage by licensed nurses assures that highest quality professional nursing care is constantly available.
  • Emergency medical care is available from Live Oak’s nurses certified in C.P.R. Live Oak provides an on-going nursing assessment of each resident.
  • A continuing program of health education is directed toward maintaining and improving the quality of life for each Live Oak health care resident.

Resident Activities

Live Oak’s Creative Corner serves as the hub for many social and therapeutic activities. It is a well-lighted, airy room adjacent to the Sharp Patio and Live Oak’s Beauty Shop. The patio is designed to encourage the residents to participate in gardening, activities and exercises, as well as allow them the opportunity to walk and enjoy the outdoors. In Creative Corner, activities are designed to assist residents in maintaining various psychomotor skills through painting, cooking, crafts, piano playing, cards, games, singing, and dancing.  

There are also off-campus activities for the residents of Live Oak. These include bus rides, visits to museums, parks, local festivals, and meals at restaurants. Of course, family members are welcome to share in the fun of all Live Oak activities, on campus or off.

Services provided by the Activity Department include:

  • Daily scheduled activities are planned each month. The monthly calendar of events, The Acorn , is sent to each family member by mail.
  • Activities are geared toward the interests and desires of each Live Oak resident. Our residents exercise great input into the monthly schedule of activites.
  • Live Oak offers specialized as well as group activities to accommodate small groups or large parties.
  • Live Oak offers specialized in-room activities for those residents who are unable to leave their room.

Counseling Services

Live Oak is unique in that we have on staff a licensed marriage and family therapist on our staff. This position organizes family support groups, grief support groups, family and individual counseling sessions, dementia / Alzheimer’s support groups, and family education seminars.

Nutritional Services

Nutritional Services The Director of Dining Services, a member of the health care team, completes a full nutritional assessment for each new resident. This discussion helps determine individual preferences for specifics foods, the presence of food allergies and recommend food supplements if necessary and appropriate.

Leisurely mealtimes are provided for residents and their guests in attractively decorated dining areas. Each resident is served well-balanced nutritious meals as prescribed by the attending physician. Residents are encouraged to enjoy their meals in the dining area where they can participate in social contact with others or, if preferred, the residents can be served in their rooms.

Social Services

Social services are available to the residents in the Health Center under the direction of a degreed Social Services Director (SSD). The SSD assesses the non-nursing needs of the resident. With families, the SSD develops a social plan of care to identify the activities which are of personal interest to the resident and will make the transition to a new life in the Health Center fun and entertaining. Services provided by the Social Service Department include:

  • Assistance with the admission process
  • Assistance with Long Term Care insurance claims
  • Conducts monthly resident and family council meetings
  • One on one visits with residents
  • Assistance with Medicare Part D enrollment and troubleshooting
  • Obtaining the use of community based services to aid resident and their families
  • Assists in achieving a high quality of life for each resident Nutritional Services

Nursing Services

The Winona Brown Ward Health Center is a long-term care facility with private and semi-private rooms each with its own full bath, cable and closed circuit community television keeping you informed of all aspects of life at Live Oak. Each room also has private phone access available.
