Admission Requirements

Each applicant, 62 years of age or older, will:

• complete an application with a non-refundable fee;
• receive a pre-admission screening for placement at Live Oak by our trained medical staff;
• also, applicant’s attending physician must complete a history and physical to prior to admission at Live Oak.
