Admission Requirements

Applicants for admission to Independent Living will complete an application and submit it with a non-refundable fee.

The applicant’s attending physician will examine and complete a history and physical to be included with the application.

Each applicant, 62 years of age or older, will receive a pre-admission screening for placement in Live Oak’s independent living area by Live Oak’s admission team.
